Before transforming its workflow, establishing an outcomes project on THE CLINICIAN PLATFORM (TCP) required a multi-week authoring life cycle. Every clinical protocol had to be manually mapped into variables, cohorts, assessments, schedules, and custom reporting logic before undergoing strict schema validation.

Because this legacy model relied entirely on scarce technical capacity, the operational friction and long timelines made smaller quality-improvement initiatives or localized registry projects difficult to justify. Conventional form wizards simply lacked the contextual reasoning capabilities required to turn natural language into validated platform configurations.

To eliminate the authoring bottleneck, The Clinician built TC-WIZ, a no-code project-authoring wizard powered by RAI (Responsible AI), their built-in Clinical Assistant. Built using Gemini on Google Cloud and Gemini Enterprise Agent Platform (formerly Vertex AI), RAI functions as an enterprise-grade assistant connected directly to the wizard’s state. Rather than behaving like a generic chatbot, RAI utilizes advanced function calling to update drafts, build form sections, and generate scoring formulas from raw protocols.

Deployed across Cloud Run functions, the architecture features a hybrid model routing pattern running nine production endpoints. It dynamically routes complex reasoning and document extraction to Gemini Pro, while assigning high-volume, latency-sensitive tasks like question drafting to Gemini Flash using the Gemini Enterprise Agent Platform. To ensure healthcare-grade compliance, TC-WIZ screens all inputs for Protected Health Information (PHI), automatically blocking calls if sensitive data is detected. Embedded licensing controls also check requested clinical instruments against curated libraries, protecting intellectual property.

The result is a governed, clinician-led experience that reduces setup from weeks to hours. By routing tasks between Gemini Pro and Gemini Flash, we maintain a responsive user experience while preserving the strict validation, licensing safeguards, and audit logging healthcare organizations expect.
